You can register with the
Practice if you live within our
Practice Area.
Our Receptionist will ask you to
complete a registration form
GMS1. If
you have your NHS medical card
please bring this along. If you
do not have your NHS Card then
please try to find out your NHS number
from the GP practice you are
currently registered with before
you come to your new patient
appointment. Having this
information will enable us to
get your medical records to our
doctors as quickly as possible.
We ask all new patients
16 years and over
who join
the practice to attend an
appointment with our Practice
Nurse or Health Care Assistant
where you will have the chance
to discuss your health and
complete the administration
necessary to register you at our
practice.
For patients joining the
practice under the age of 16
years we request that a
New Patient Questionnaire
and a
GMS1 is completed for
each young person and brought in
to the practice when the adult
family members come for their
registration appointment. We
also request that all adults
registering bring 2 forms of ID
(one photo ID) with confirmation
of their name and current
address to their registration
appointment. It would
also be helpful if you have a
record of any repeat medication you
take and any immunisations that
you have had to bring that along
to the appointment.
If you are unable to attend your
booked New Patient Appointment
then please telephone 0161 483
5155 during surgery office hours
to re-schedule it. You will not
be fully registered at our
practice until
you have completed this process.
Once registered with the
Practice you may choose which
doctor you see each time you
make an appointment.
